AWARD FORMS
1- The Chapter Reports Form : to be preferably filled by your SOYO President and reviewed by your kind self. Please make sure that the report does not have the parish name elsewhere than on the cover page,
2- The Most Active Teen Nomination Form : to be filled by the Parish Priest with the help of the Youth Advisor when applicable. Same concept, please do not mention the name of the teen other than on the cover page.
